RECERTIFICATION PATIENT (TELEHEALTH)
CURRENT CCC PATIENTS- Use the RECERTIFICATION button to make your appointment.

NEW/TRANSFER PATIENT: Use the ALL NEW/TRANSFER button, fill out your intake form, set your in-person appointment .

*Recertifications required each 210 days by State Law.
***Trip charge for in-home visit is $200/person additional.
State Card Fee $77.75 directly to State, good for one year.
****All fees plus credit card processing. Payment must be received to hold your scheduled appointment time.
A. -Due to the new Florida State MMJ Limits as of 8/29/2022; there are daily caps on each route and a total cap of 24,500 per 70 day period ,excluding smokeable flower (see attached Document).
At your appointment, you will be asked your preferred routes and amounts for each route.
If after your appointment you need to make any changes to your current certification there will be a $25 administrative fee to make those changes per change request.
We would like you to avoid these extra charges in the future for the extra administrative time, please tell Dr. Weisman “All The Routes” that you desire at the time of your appointment.
B.-Per Emergency Rule 64ER22-8(6)(b), which went into effect on 8/29/2022, a qualified physician who accepts a qualified patient with an existing certification for an approved exemption request from their previous qualified physician, will have seven (7) calendar days to close the patient’s existing certification, including closing of the approved exemption request. After the seventh day, all certifications from the patient’s previous doctor with an approved exemption request will be automatically closed.
In rare instances, at the Dr.’s discretion, it may be necessary to petition the state to increase specific route amounts above the set maximum and/or smokeable flower to 4.0 oz. An RFE
( request for exemption form) may need to be filed. At each recertification, a new RFE must be submitted.
An RFE must be done for a transfer patient with a pre-existing RFE and when there is a new certification that is above the state limits for routes or smokable flower.
There is a $50 administrative fee for filing an RFE exemption form for each request.
Unfortunately, there is no guarantee that the RFE will be approved by the state, it is reviewed by them on a case by case basis and they determine approval or denial, therefore there will be no refunds for exemption denial by the state, as that is out of our control.
